Ep-CAM Background

 

 

Ep-CAM (Epithelial Cell Adhesion Molecule), also known as CD326 or TROP1 (Trophoblast Cell Surface Antigen 1), is a type I transmembrane glycoprotein encoded by the TACSTD1 (GA733-2) gene with a molecular weight of approximately 40 kDa. EpCAM not only functions as a cell adhesion molecule but also participates in the regulation of signaling pathways, promoting tumor progression. It is primarily expressed at low levels in normal epithelial tissues (such as the gastrointestinal tract, breast, and pancreatic and biliary ducts); it is significantly overexpressed (>80% of cases) in epithelial tumors (such as breast, colorectal, pancreatic, and ovarian cancers); and is generally negative in non-epithelial tumors (such as sarcomas and lymphomas), making it an ideal therapeutic target.

In the field of tumor treatment, Ep-CAM target research is applied to antibody-drug conjugates (ADC), bispecific antibodies (BsAb), and CAR-T cell therapy.

Ep-CAM and tumor diagnosis

Epstein-Barr virus (EpCAM) is positively expressed in the vast majority of adenocarcinomas and neuroendocrine tumors, making it an important marker for diagnosing epithelial tumors. Epstein-Barr virus (EpCAM) IHC is commonly used for:

1. Determine the epithelial origin of a tumor (e.g., distinguishing poorly differentiated carcinoma from sarcoma/lymphoma).

2. Recommended antibodies: MOC-31 (high specificity), Ber-EP4 (high sensitivity).

3. Assist in identifying the primary site of metastases (e.g., EP-CAM+ liver metastases may originate from gastrointestinal/breast primary cancer).

In addition, Ep-CAM can be used as a prognostic marker for epithelial tumors. EP-CAM promotes tumor invasiveness by activating the Wnt/β-catenin and c-Myc pathways and is closely associated with the survival of cancer patients.
